I request under the Act, the following information:

1] The WHO Collaborating Centre for Research and Training in Mental Health and Section of Mental Health Policy lists a number of “Honorary Members” on its website:

a) Is Dr Richard Sykes currently an “Honorary Member” of the WHO Collaborating Centre?
b) From what date did Dr Sykes’ appointment commence?
c) If Dr Sykes is no longer an “Honorary Member”, on what date did this appointment cease?
d) Through what process was Dr Sykes appointed an “Honorary Member”?
e) In his capacity as an “Honorary Member”, what roles and responsibilities has Dr Sykes held in relation to the WHO Collaborating Centre? (please give dates)
f) Does Dr Sykes receive any remunerations, honoraria, grants, awards or funding from the Institute of Psychiatry/King’s College London/WHO Collaborating Centre or any other institution, organisation, body or agency in relation to his Honorary Membership of the WHO Collaborating Centre? (please specify)

2] On the website of the WHO Collaborating Centre, Dr Sykes’ is listed as Co-ordinator of the CISSD Project. When authoring research papers and reviews (including the Review Paper resulting out of the CISSD Project published by the three Project leads in July 2007) Dr Sykes’ address is given as “WHO Collaborating Centre, Institute of Psychiatry, University of London, UK”.

a) What is the relationship between the CISSD Project and the WHO Collaborating Centre?

3] According to proposals for ICD-11 Revision submitted by Dr Richard Sykes in March 2008, via the WHO ICD Update and Revision Platform, Professor Rachel Jenkins acted as “Principal Collaborator” for the CISSD Project; the Project is understood to have been undertaken between 2003 and 2007:

a) From what date has Professor Rachel Jenkins had an involvement with the CISSD Project?
b) From what date was Professor Jenkins appointed as “Principal Collaborator” for the CISSD Project?
c) What has Professor Jenkins’ involvement been in the CISSD Project in her capacity as Director of the WHO Collaborating Centre and “Principal Collaborator” for the Project?

d) By what date was the CISSD Project Work Group assembled?
e) Through what process were members and chairs of the CISSD Project Work Group appointed?
f) If Terms of Reference were drawn up for the CISSD Project, I request a copy under the Act.
g) Were CISSD Project Work Group members and chairs required to complete Conflict of Interest Disclosures?

h) In submitting proposals to the WHO ICD-11 Update and Revision Platform, is Dr Sykes acting on behalf of the CISSD Project Work Group in his capacity as Co-ordinator of the Project, or acting on behalf of the WHO Collaborating Centre in his capacity as an “Honorary Member” of the WHO Collaborating Centre, or acting in a personal capacity?

4] Other than the Review Paper: Revising the Classification of Somatoform Disorders: Key Questions and Preliminary Recommendations, Journal of Psychosomatics; 48:277-285 published by the three CISSD Project leads in July-August 2007, were interim reports and/or a final report provided to the WHO Collaborating Centre by the Project?

a) I request copies under the Act.

5] Under the Act, I request complete copies of correspondence between the WHO Collaborating Centre and the CISSD Project since the Project’s inception.

This to cover all forms of communications between the organisation and the Project since the Project’s inception: to include copies of letters, emails, faxes, reports, agendas, minutes, summaries or notes of all meetings and Project Work Group meetings held in the UK and US, whether attended by Professor Jenkins or not, and reports, briefing and presentation material prepared for or connected with meetings and meetings of the Project Work Group, either before or after the event.
